What to bring to your first appointment

Please be sure to bring all relevant documents with you to your first appointment. If not, you will have to pay the full amount out of pocket and will not be able to attend the clinic. Thank you for your understanding.

Health insurance certificate：In case of an extraordinary event, please make sure to bring.

According to the existing health insurance system, healthcare providers are required to register the patient's health insurance card once a month.
Depending on the type of your health insurance card, the amount of insurance reimbursement will change.
Please bring your health insurance card with you in order to prevent special events, such as changing the type of insurance, exceeding the amount of insurance, etc. Thank you for your cooperation.
Please notify us even if your health insurance card expires, your address changes, etc.
If you forget to bring your health insurance card on the day of your visit, we will charge you the full amount.

About the process of the initial visit.

STEP 1

Reservation

We operate a prior appointment system.
(We also cater for emergency cases, please call for details)

While inquiring about an appointment, please briefly describe your symptoms and provide information such as your personal name, phone number, whether you have a certificate of insurance or not, and the exact time of your visit.

STEP 2

Completion of enquiry form

At the initial consultation, please be sure to arrive 10-15 minutes before your appointment time as we need to collect your information, log in your insurance card, etc.
We will keep your insurance card for a while (it will be returned at the end of the consultation when the bill is settled). Please fill in as much detail as possible about your symptoms on the consultation form.

STEP 3

Q&A

We will follow the contents of the enquiry form you have completed.
Describe the tooth pain to the doctor as much as possible to facilitate the doctor's diagnostic and treatment judgement.

STEP 4

Description of treatment

The specific treatment plan will be explained to the patient on a case-by-case basis. Once the patient's understanding is obtained, treatment will begin.

If special treatment options are available, e.g. porcelain teeth, dental implants, etc., it is up to the patient to decide on the treatment plan.

Note: According to the Japanese health insurance system, an oral cleaning is required prior to treatment for worm teeth etc. (Tartar removal, tartar removal, etc.).

STEP 5

Regular inspection

After all treatments have been completed, depending on the condition of the patient's teeth, we recommend regular (3-6 months) cleanings as well as simple check-ups.
In case of orthodontics, regular check-ups are mandatory. Please understand.

Transport Route

Directly accessible from Seibu Ikebukuro Line and Toei Oedo Line (2-minute walk from the battlefield)
We have a parking lot for patients only, but the number of parking spaces is limited. Please use public transportation whenever possible.

Using Toshimaen Station of Seibu Line

①Exit the station and go to the right hand side.
②After walking straight for about 100 metres, proceed to the right at the intersection with a 711 convenience store on your left. (No need to cross the zebra crossing)
③Walk for about 100 metres and you will see the Matsuya on your right hand side. Next to the Matsuya is the main hospital.

Using Toshimaen Station of Oedo Line

①After passing through the underground ticket gate, please leave the station through Exit A2.
②After leaving the station at Exit A2, go straight to the right.
③Walk for about 100 metres and you will see "Matsuya" on your right hand side. Next to "Matsuya" is our hospital.
